Working for HMS Care as a Care Coordinator means you will be part of one of the North East’s leading providers of home care.
The HMS Care Coordinator is responsible for ensuring that all of the care team are at the right clients homes’ at the right times’ for the correct duration. This is planning at its most critical as it directly impacts on the quality and continuity of the care that clients receive.
The most important factors as far as our customers are concerned, are to have care visits from people who know them and who turn up for the care visits when they are expected to. In any given region the Care Coordinator supports the Care Supervisor to ensure that this is actively planned to be seamlessly delivered. It is essential to have an analytical flare for problem and puzzle solving to be considered for this role with HMS Care. You must have exceptional IT, communication, planning and organisational skills to work as a coordinator, combined with the ability to perform in a high pressure environment.
To ensure that ALL of our customers receive care in line with their high expectations you will:-
- Organise care worker rotas to minimise changes to the service and travel between customers’ homes
- Allocate care workers with the right skills and experience to deliver the best quality care and support to each customer
- Respond to unforeseen daily dynamic changes to schedules, such as dealing with last minute carer absenteeism or a customer who falls ill and requires an extended visit
You are the conduit between the customer and the care team. You will deal with care queries from customers and care team issues such as traffic problems. Your telephone manner and communication skills must be calm, understanding and reassuring to ensure that every stakeholder in the HMS team knows where they should be and what is required of them.
You must also be a great motivator of people as you will often need to ask the care team members to cover extra shifts at short notice.
The HMS care team work over the weekends and evenings (on shift basis 365 days per year) so the post holder must be willing to be part of the on call emergency team rota.
